"About 20% of individuals with Corona Virus disease (COVID-19) experience more severe disease characterized by significant respiratory symptoms including acute respiratory distress syndrome (ARDS). ARDS is a known lethal complication due to its low blood oxygenation levels and may result in organ failure. Until now, there are no specific vaccines or therapeutic drugs targeting SARS-CoV-2, alternative therapeutic interventions are needed to prevent and ameliorate respiratory conditions associated with COVID-19 to effectively reduce mortality and prevent ICU admissions. Aviptadil, which is a synthetic form of Human Vasoactive Intestinal Polypeptide (VIP), might be beneficial in patients at risk of developing ARDS. Nonclinical studies demonstrate that VIP is highly concentrated in the lung, where it prevents N-methyl-D-aspartate (NMDA)-induced caspase-3 activation, inhibits IL-6 and TNFa production and protects against HCl-induced pulmonary edema. Further, in animal model systems of lung injury in mice, rats, guinea pigs, sheep, swine and dogs, Aviptadil was shown to restore barrier function at the endothelial/alveolar interface and to protect the lung and other organs from failure. In Europe, Aviptadil is approved for human use and has been shown to be safe in phase II trials for sarcoidosis, pulmonary fibrosis, bronchospasm, erectile dysfunction as well as in a phase I trial in ARDS in the past two decades. In the US, VIP has been given FDA Orphan Drug Designation for the treatment of ARDS and was admitted to the FDA Corona Virus Technology Accelerator Program. In a phase I trial of Aviptadil performed by Sami Said in the early 2000s, eight patients with severe ARDS on mechanical ventilation were treated with ascending doses of intravenous VIP. Seven patients (88%) were successfully extubated and were alive at the five day time point. Six (75%) left the hospital and one (13%) died of an unrelated cardiac event. A phase II clinical trial using intravenous Aviptadil in patients with COVID-19 infection and ARDS has begun. Further, a phase II/III clinical trial will study the effect of inhaled Aviptadil for the treatment of non-acute lung injury in COVID- 19 and begins in June 2020. In Europe, two phase II trials of Aviptadil have been conducted. Further, studies with healthy volunteers have shown that inhaled Aviptadil is well tolerated with few adverse effects."

Ik zit behoorlijk te balen omdat het me maar niet lukt een aantal van die genoemde onderzoeken te vinden maar alles wat ik wel vind ziet er toch behoorlijk goed uit. Ik vond wel een oud FDA verhaal waarin iemand vond dat er toch negatieve bijwerkingen waren terwijl iemand anders vond dat die niet door VIP kwamen. Dat kan enige voorzichtigheid bij de FDA wel verklaren.

Ik kan je niet volgen voor wat betreft het laatste gedeelte waarin je zegt dat de FDA voorzichtig zou zijn inzake VIP. Ik hoop dat je dat niet projecteert op de huidige goedkeuringsprocedure? Hoe bedoel je dat? FDA heeft Aviptadil al expanded use gegeven en een geschreven verklaring uitgebracht dat het veilig is.

Vergeet niet dat de EUA aanvraag slechts uitbehandelde patiënten betreft, dus als Dexa en Rem er al doorheen zijn gegaan...

Dat het zo lang duurt is dus omdat de FDA geen haast heeft door het gebruik van Aviptadil al te hebben toegestaan. Verder zullen ze enorm bezig zijn met de vaccinsmakers als Moderna en nu ook met Regeneron. Beide producenten hebben meer dan een half miljard gekregen! Vandaar ook dat Trump er nu zo op hamert toch vooral EUA te verstrekken aan Regeneron. Immers, als het niet snel de markt haalt heeft ie overheidsgeld gezet op een loser…en bovendien is het troefkaart voor de verkiezingen geworden – ook door zijn Covid-19-genezing.

Hier een aantal opmerkingen over wat er nu speelt - gevonden op diverse fora:

Former FDA Commissioner says the current treatments and antibodies we now have is nowhere near enough to treat everyone and it will take months to get up to speed. RLF-100 is available now in large quantities. Blames government on dropping the ball, they should have started production in May.
If we become an option, RLF-100 could fill in the gaps. This is why we are gearing up for mass production and making partnerships with companies that can produce it.

I believe that the pressure put on getting EUA for Regeneron could help speed up RLF 100 EUA. Indeed it’d be difficult to justify giving in 5 days to one what you don’t give in 30 days to RLF 100. So a proper cover up would be to authorize several new therapies in a row... and don’t forget team.. not long ago everyone was freaking out with vaccinez.. Now therapies had their strong “marketing”.

On July 6 Regeneron received almost half a billion dollars from OWS to immediately start production of its MCA cocktail. Of course that was well before the safety or the efficacy of Regeneron's drug had been demonstrated. In retrospect do you think this might be an example of Trump Derangement Syndrome?

Former FDA head Scott Gottlieb on Face The Nation is on record saying the career bureaucrats in the FDA & CDC failed Trump by over promising & under delivering. Also, he said that the Agencies underestimated the disease & overestimated their ability to respond to it.

When President Trump promoted an experimental drug as a “cure” for Covid-19 in a video on Wednesday, it might have seemed that he was at it again: touting a questionable fix for a deadly pandemic, not unlike his earlier enthusiasm for the malaria drug hydroxychloroquine or even, at one point, disinfectant.
But the treatment that Mr. Trump extolled, which was administered last week after doctors diagnosed Covid-19, is not a fringe product. It’s a promising drug in the final stages of testing developed by a respected biotech company, Regeneron. Infectious disease experts have been closely following the treatment, as well as a similar product from Eli, in the hopes that the therapies could be a real advance in the fight against Covid-19.
Pharmaceutical companies often pay handsomely for celebrity endorsements, but this patient testimonial was like no other. It came from a polarizing president who, just weeks away from an election, and having found himself and his White House at the center of an outbreak, is eager to show that his administration is doing something about a pandemic that has killed more than 212,000 Americans.

Although he couldn’t possibly have known whether Regeneron’s treatment had helped him — or even if he was out of the woods yet — Mr. Trump sang its praises in the video, calling it “unbelievable” and suggesting it was only moments away from being authorized it for widespread use. In doing so, Mr. Trump reminded his critics of the many times — from reopening schools to authorizing hydroxychloroquine and blood plasma — over the past nine months that he has inserted politics into the decisions of independent health agencies.

Regeneron, which filed an application with regulators within hours of the president’s video, must now shepherd its antibody treatment through a politically fraught approval process, where the president’s over-the-top endorsement has likely raised the profile of its product, but could also sow suspicion about whether it works.
“I don’t see how it is going to end up being good for a pharma company,” said Ronny Gal, a pharmaceutical analyst for the Wall Street firm Bernstein. “Once you become a political opinion, that’s not great.”
Already, Regeneron is fielding messy questions about how its treatment was tested using cells originally derived from an aborted fetus — a line of research that Mr. Trump has opposed — and the president’s relationship with Regeneron’s chief executive.

Mr. Trump has further complicated the potential rollout of these treatments by pledging — first on Wednesday and again in another video Thursday — that the drugs would be free of charge and would be soon be available in hundreds of thousands of doses.

De laatste stand van zaken in zake Covid-19 is dat er duidelijke aanwijzingen zijn dat een lage begin hoeveelheid virus zorgt voor een minder ernstig verloop. Remedisvir, HCQ en Regeneron lijken daardoor in de begin fase enig effect te hebben en later eigenlijk niet meer. Dit gaat denk ik toch effect hebben op de verkochte hoeveelheid RLF100 via infuus, niet dat het overbodig zal zijn want er zullen altijd mensen op de IC belanden door Covid-19 zolang mensen niet immuun zijn. Er zijn geen aanwijzingen dat er een minder gevaarlijke Sars-Cov2 variant is ontstaan.

President Donald Trump, ahead of anticipated rallies in Florida as soon as Saturday, went virtual with a radio showcase Friday on the Rush Limbaugh show.

The big news: “We have a cure” for COVID-19, he told the conservative talk master.

“We have a cure, some call it a therapeutic. I call it a cure that most have never heard of that’s going to be out very soon,” Trump assured Limbaugh in the noon hour.

The President was referring to plans announced to provide over a million doses of the same treatment the President got, pending approval by the Food and Drug Administration.

“I was in not great shape, but we had a medicine that healed me,” Trump said of the Regeneron antibody cocktail he was given at Walter Reed Hospital this week.

That cure will be widely available soon, Trump asserted.

“Hundreds of thousands of vials are being sent to hospitals all over the country.”

“We can go into hospitals and clean up the hospitals,” Trump said. “I had a meeting with the doctors today. These eleven guys, they showed me stats, it was amazing.”

“We’re sending that to all our hospitals,” Trump said. “This is stuff that’s so good it wiped out the virus.”

The President continued to extol his own health.

“I’m free. I feel perfect. I’m not taking anything.”

“You sound great,” Rush said.

“My voice is now perfect,” Trump said. “This is the most amazing thing that has happened.”

Doctors told him he was “going into a really bad phase,” and Trump noted that he’d lost five friends to the virus.

“Without us. Without the Trump administration, this never would have happened,” Trump added.

The President and the host revisited the topic in the second hour of the program, with Trump continuing to wax optimistic.

“This is a total game changer, it’s so good,” Trump said, noting that the “FDA is approving things in one week that would have taken three years … and they’re great things.”

“This is the greatest of them all if it works out,” Trump added.

The President has expressed similar confidence in his recovery in other recent interviews.

Speaking Thursday night on Fox News Channel, Trump said he was ready to get back on the campaign trail, and feels “perfect” just one week after his COVID-19 diagnosis.

A possibility of this company being one of two pharmaceutical companies who have the cure President Trump mentioned in the Rush Limbaugh interview.

Dat gedoe over Trump vind ik z'n onzin. Stukjes gehoord van zijn interview, hij springt van de hak op de tak, zegt vreemde dingen en noemt geen namen behalve Regeneron en lilly. Waar hij heel blij van is geworden is volgens mij duidelijk Dexamehason en dat begrijp ik best, hij begrijpt helemaal niets van de werking van de medicijnen. Als mensen graag willen horen dat hij RLF100 bedoelt prima maar het is volgens mij volledige onzin. RLF100 werkt langzaam bij een flinke infectie, Dexamethason geeft meteen energie en het kan zorgen dat je je geweldig voelt, dat beschrijft Trump.

k ben een medische professional. Ik denk dat leken te veel bezig zijn met de lekenpers en bang zijn dat Aviptadil op de een of andere manier over het hoofd zal worden gezien. Sommigen vrezen zelfs dat de pandemie al voorbij is of zal eindigen voordat Aviptadil zal worden goedgekeurd. Ik kan iedereen verzekeren wat alle medische professionals al weten. De veiligste en meest effectieve medicijnen zullen altijd degene zijn die worden gebruikt. De FDA zal uiteindelijk naar de waarheid worden beoordeeld. Als ze door een of andere snode motivatie de goedkeuring van Aviptadil vertragen of weigeren, zal dat uiteindelijk aan het licht komen. Als de goedkeuring van Aviptadil hier wordt uitgesteld, maar in plaats daarvan in Europa of Azië als eerste wordt gebruikt, zullen die feiten bekend worden. De mensen die de goedkeuring hebben vertraagd, zullen worden belasterd en aan de schandpaal worden genageld. De FDA weet dit. De goedkeuring kan weken of zelfs een maand of zo worden uitgesteld, maar dat is alles. Als dat te veel is voor een investeerder die er is, ze zijn overbelast en zouden hun positie geheel of gedeeltelijk moeten verkopen. Het maakt niet uit hoeveel men de melk schudt, ongeacht de onrust, room stijgt altijd naar de top. De wetenschap zal altijd winnen. Op de lange termijn maakt het eigenlijk niet uit welke farmaceutische bedrijven betere PR hebben.

Geduld houden hoe moeilijk het soms ook is en dan ook nog is alle media aandacht voor andere medicijnen die erbij komt. Hou deze parel vast, het gaat worden beloond, corona is vooelopig nog niet voorbij.

Ja grappig en ironisch dat Hydrochloroquine nu een beetje terug lijkt te zijn van weggeweest als middel dat toch een beetje helpt aan het begin van de behandeling.
Ik geloof dat enkel op de korte termijn hier marktaandeel wordt opgeëist. Het beste medicijn laat zich niet verloochenen.

Verder is het virus inderdaad niet aan het verminderen op wereldschaal. Het zou wel eens een zware winter kunnen worden.

Volgens mij bedoelt ie Regenerons middel, maar was het Dexa dat het werk deed in zijn lichaam. :) Zoals eerder beargumenteerd ligt het niet voor de hand dat Trump voor VIP zou gaan.

Trump heeft altijd een dubbele agenda.

If you are somehow mad with FDA, here is a little timeline for the relevant therapeutics seeking EUA:

Gilead: Interim data (391 Remdesivir + 340 placebo) readout 04/27/2020. EUA granted 05/01/2020

Lilly (1): Interim data (302 monotherapy + 150 placebo) released on 09/16/2020. EUA applied 10/07/2020
Lilly(2): Interim data (112 cocktail + 156 placebo) released on 10/07/2020. EUA submission planned November 2020

Regeneron: Interim data (275 total, high dose cocktail: low dose : placebo 1:1:1) released on 09/29/2020. EUA applied 10/08/2020

Relief: Open label EAP data (21 Aviptadil + 30 controlled) released on 08/20/2020, EUA applied 09/17/2020 (09/23/2020 PR)

If FDA is satisfied with our EAP data, then we are ahead of both Lilly and Regeneron for a limited EUA. If FDA wants our 102 interim data, we are one month late compared to those two. But most likely we will get a full EUA in this case if the data is good. Either way FDA is well within its time frame to make a decision. Let them do their job. We will be fine.

Oneens. Als het goed bespreken ze de 'som' van wat er is. De goede verstaander zou daar dingen kunnen horen die direct of indirect betrekking hebben op Relief.

Eens in de zin dat we niets horen over EUA.
